Grandma on a saddle. Grandpa with a cane strokes a horse. It sounds strange to many. Hippotherapy for the elderly is a reality. It helps with arthritis, dementia, depression, and recovery from strokes. A horse is a trainer, a psychotherapist, and a friend. In 2026, this method is actively used in Europe and is beginning to develop in Russia. We tell you how it works.

What is Hippotherapy

Hippotherapy is a rehabilitation method using horses. The term comes from Greek «hipos» — horse. Do not confuse it with therapeutic riding (TR), although these concepts are often mixed. Hippotherapy is conducted by specially trained instructors (often with a medical education) and doctors.

The horse transmits thermal and mechanical impulses to the rider. The rhythmic swaying (gait of the horse) massages the muscles of the back, legs, stimulates blood circulation, and improves the function of internal organs. The rider has to maintain balance — this trains the vestibular apparatus.

For the elderly, this is especially important because with age, balance decreases, muscles weaken, and coordination deteriorates. The horse gently, without jerks, makes the body work.

The psychotherapeutic effect: a horse is a living, warm, non-verbal creature. Elderly people suffering from loneliness get contact. Their cortisol level (a stress hormone) decreases, and their mood improves.

How sessions are conducted

The session lasts 20-30 minutes. The elderly person is seated in a special saddle with back support. The instructor leads the horse by a halter (on a long rope). The speed is a walk. Sometimes a second assistant is involved — supports the rider from the side.

Activities can be passive (the elderly person simply sits, relaxed) and active (performs exercises: raises hands, turns the torso, bends over). For people with dementia, just pet the horse, feed it by hand. This stimulates tactile sensations and memory.

For the elderly with amputated limbs — special attachments on the saddle to prevent the person from falling.

Important: a consultation with a doctor is mandatory before starting (therapist, neurologist, orthopedist). Contraindications: acute inflammation, thromboses, unstable fractures, severe hypertension, epilepsy.

Activities are conducted 1-2 times a week. The course is 10-20 sessions.

What Hippotherapy Treats in the Elderly

Orthopedic diseases: arthritis, arthrosis, osteochondrosis, osteoporosis. The horse gently rocks the joints, improves mobility. Elderly people with Parkinson's disease train balance, the progression slows down.

After a stroke: recovery of motor functions, reduction of spasticity (muscle tension). Patients who could not walk began to move with crutches after 2-3 months of training.

Dementia (Alzheimer's disease and others): cognitive decline slows down. The horse stimulates emotional memory. The elderly start to smile more often, remember (for example, how they rode horses as children).

Depression, loneliness: the horse gives unconditional love. Many elderly people cry on the first session and then thank. Communication with an animal reduces the risk of suicide.

Hypertension: studies show that after 3 months of hippotherapy, blood pressure drops by 10-15 mmHg.

Marina Ivanovna, 78 years old, had a stroke six months ago. The left hand and leg almost did not move. After 12 sessions (3 months), she began to stand up with support, the left hand raises to the shoulder. The doctors were surprised. Marina Ivanovna says: «The horse pulled me out.»

Alexey Petrovich, 85 years old, Alzheimer's disease. He did not recognize his grandchildren. After 8 sessions, he started to pet the horse, call it «Zorya» (although the horse was called Groza). Where the Elderly Practice Hippotherapy

In Europe, every rehabilitation center has a «stables». In Germany, hippotherapy for the elderly is paid for by insurance companies (partially). In France, state programs.

In Russia, hippotherapy for the elderly is still rare. The main centers: Moscow (Bitca stable, Live Thread center), St. Petersburg (Horse and Life center), Kazan, Ekaterinburg, Novosibirsk. But mainly this is commercial centers, the price is 1500-3000 rubles per session.

In 2026, a program called «Accessible Hippotherapy» was launched in Russia for pensioners and disabled people. Under it, you can get up to 5 free sessions by doctor's referral. But there are few centers, queues.

How to Convince an Elderly Parent to Try

Who Hippotherapy is Contraindicated for

Absolute contraindications: unstable vertebral fracture, acute thrombosis, aneurysm (risk of rupture), active oncology, severe dementia (the person does not fix the gaze, does not understand where they are).

Relative (after consultation with a doctor): epilepsy (if seizures are rare), stage 3 hypertension (high risk of stroke), stage 4 obesity (horses are heavy), joint diseases (can, but with caution).

Important: before starting the sessions, the doctor must assess the stability of the condition. If the elderly person recently had a heart attack, you need to wait 6 months.

Horse for the Elderly: Myths and Reality

Myth: a horse can kick, bite. Reality: the most peaceful horses are selected for hippotherapy (often mares, retirees). They are used to people, even to incoherent ones. A horse bite is an extremely rare case.

Myth: it's easy to fall. Reality: the horse walks at a walk, the rider is supported by two assistants, a saddle with a high back. Falls are excluded (but not 100%). Statistics: over 20 years of hippotherapy in Germany — 0 fatal cases.

Myth: it's expensive. Reality: yes, expensive. But there are compensation programs. Moreover, it is more effective than some medications. Plus — free emotions.

Myth: it's only for children. Reality: hippotherapy for the elderly is even more effective (fewer contraindications than in children with severe pathologies).

Old age is not a disease. But it brings pain, weakness, loneliness. The horse helps to forget about age. In 2026, when life expectancy is increasing, hippotherapy becomes a necessity. If you have an elderly relative who suffers from arthritis, dementia, or just loneliness, take them to the stable. Let them breathe the smell of hay, pet the warm face. Perhaps it will change their life.

The horse does not ask for age. It gives joy. And this is the main medicine.
